3 july 2011 - Otmuchów. SM42-2164 jako popych ładownego składu prowadzonego przez M62M-002 - jadą w stronę Nysy. Tego dnia nastąpiła awaria rogatki i nastawniczy miał sporo pracy, biegając do przejazdu - zanim usunięto usterkę.
